2026 Forvis Mazars Home Health Medicare Payment Rates 

Forvis Mazars is pleased to provide the members of the Council of State Home Care & Hospice Associations (Council of States) an analysis of home health Medicare payment rates in 2026 compared to 2025.  These rates are for each Core Based Statistical Area (CBSA) in each state and provide a comparison of the 2026 versus 2025 home health Medicare rate, prior to consideration of the patient-specific Patient Driven Groupings Model (PDGM) case-mix weight and home health agency-specific Value-Based Purchasing (VBP) rate adjustment.

These rates are intended as a Council of States benefit that you may share with your association’s individual members and can be distributed in any means you feel would most benefit your members, including posting to your association’s website.
